About The Position

Join an established, full-service family medicine practice caring for patients of all ages, from infants through adulthood. CapitalCare Family Practice Great Oaks offers a collaborative team environment, flexible scheduling, and the resources of CCP's physician-led multispecialty network. This role involves providing comprehensive primary care, including preventive, acute, and chronic disease management, and building meaningful, long-term relationships with patients and families.

Requirements

  • Current New York State Nurse Practitioner or Physician Assistant license
  • Graduate of an accredited NP or PA program
  • Strong clinical, communication and interpersonal skills
  • Commitment to patient-centered, compassionate care
  • Physician Assistant: Bachelor's or equivalent degree in physician assistant studies and successful completion of physician assistant training program approved by the NYS Department of Education or accredited by the Accreditation Review Commission on Education for the Physician Assistant, or a program that is determined by the Department to be the substantial equivalent of a licensure qualifying or accredited program.
  • Physician Assistant: Attainment of passing grade on the Physician Assistant National Certifying Examination administered by the National Commission on Certification of Physician Assistants.
  • Physician Assistant: Current certification by National Commission on Certification of Physician Assistants.
  • Physician Assistant: At least one year of experience as a physician assistant in ambulatory health care.
  • Physician Assistant: Current CPR certification.
  • Nurse Practitioner: Successful completion of and graduation from a nurse practitioner education program acceptable to NYSED.
  • Nurse Practitioner: Current certification as a nurse practitioner by the American Academy of Nurse Practitioners Certification Board (AANPCB) or the American Nurses Credentialing Center (ANCC).
  • Nurse Practitioner: Successful completion of pharmacology coursework acceptable to NYSED.
  • Nurse Practitioner: At least one year of experience as a nurse practitioner in ambulatory health care.
  • Nurse Practitioner: Current CPR certification.

Nice To Haves

  • Family medicine or primary care experience preferred
  • New graduates with a strong interest in family medicine are encouraged to apply
  • Three years of professional nursing experience in an ambulatory setting preferred.

Responsibilities

  • Provide comprehensive primary care to patients across the lifespan
  • Perform physicals, wellness visits, acute visits and follow-up care
  • Evaluate, diagnose and manage acute and chronic conditions
  • Provide preventive health counseling and patient education
  • Coordinate care with specialists and other members of the healthcare team
  • Utilize telemedicine when appropriate
  • Maintain accurate and timely electronic medical records
