About The Position

Ascension Wisconsin is looking for a Family Medicine Nurse Practitioner for AMG Waukesha Primary Care. This is a great opportunity for the right candidate to partner with Ascension Wisconsin to provide medical services to patients.

Requirements

• Registered Nurse credentialed from the Wisconsin Board of Nursing obtained prior to hire date or job transfer date. Licensure from the Wisconsin Board of Nursing OR current home state license if considered multi-state/Compact State. Verified by local Credentialing Office.
• BLS Provider obtained within 1 Month (30 days) of hire date or job transfer date. American Heart Association or American Red Cross accepted.
• Nurse Midwife credentialed from the Wisconsin Midwives Advisory Committee obtained prior to hire date or job transfer date. Verified by local Credentialing Office.
• Master's or Doctorate of Science in Nursing or Midwifery or Doctorate of Nursing Practice required.

Responsibilities

• In collaboration with a physician, provide for the expansion of individualized primary healthcare services by diagnosing and treating certain acute or chronic health conditions.
• Obtain patient history and perform assessment via observation, interview and examination. Order, perform and interpret diagnostic studies. Perform preventative health assessments, screening, immunizations and patient care.
• Provide direct treatment and management of health conditions via referral to other healthcare providers or community resources.
• Counsel and educate patients and their families/caregivers concerning preventative health, treatment options and community resources.
• Document delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy.
