About The Position

We are seeking a Family Medicine Physician or Internal Medicine-Pediatrics (Med-Peds) Physician to join our growing team in Northfield, NJ. Requirements

MD or DO from an accredited medical school.
Board Certified or Board Eligible in: Family Medicine or Internal Medicine-Pediatrics (Med-Peds)
Active New Jersey medical license (or ability to obtain prior to start).
DEA license and CDS registration (or ability to obtain).
Strong commitment to patient-centered, culturally competent care.

Nice To Haves

Experience working in diverse or underserved communities.
Experience in outpatient primary care settings.
Value-based care or population health experience a plus.

Responsibilities

Provide outpatient primary care services for patients across the lifespan (Family Medicine or Med-Peds scope).
Diagnose and treat acute and chronic medical conditions.
Conduct annual wellness visits, preventative screenings, and routine physicals.
Develop individualized treatment plans focused on long-term health outcomes.
Counsel patients on preventative health, lifestyle changes, and disease management.
Promote continuity of care through proactive follow-up and patient engagement.
Build strong relationships with patients and families within the local community.
Collaborate with Advanced Practice Providers, Care Managers, Behavioral Health, and specialty teams.
Coordinate referrals, diagnostics, and follow-up care as needed.
Participate in a team-based, patient-centered care model.
Maintain accurate and timely documentation in the EHR system.
Ensure compliance with all clinical, regulatory, and quality standards.
Support quality initiatives and value-based care programs.
