About The Position

Family Connections Consultants will implement the Family Connections (FC) model and curriculum with fidelity and follow all program and agency policies and procedures. This role involves conducting home visits, managing a caseload, developing service plans, and connecting families to resources. The consultant will also be responsible for maintaining case records, collaborating with various stakeholders, and reporting suspected child abuse or neglect. Additionally, the position requires training in and utilization of the Nurturing Parenting Program practice model, including conducting parenting sessions and case closure documentation. The role emphasizes a commitment to excellence and continuous quality improvement.

Requirements

  • Minimum of a Bachelor’s Degree in Social Work or related field.
  • At least one (1) year of relevant experience.
  • Knowledge of community resources, case management, assessment and service planning, and problem solving with complex family needs.
  • Understanding of DCS systems is preferred.
  • Ability to work under pressure, meet deadlines, and set priorities.
  • Well-organized and able to effectively manage multiple cases.
  • Ability to work independently and as part of a team.
  • Flexibility and ability to adapt to changing working conditions.
  • Sensitivity to cultural differences within the agency and community.
  • Fluent in Microsoft Office Applications.
  • Intermediate-level technology and software use for documentation and casework.
  • Must be able to stoop, bend, squat, sit on floor and ambulate over uneven floor surfaces including stairs and steps.
  • Ability to visually and auditorily assess and interact with clients to ensure their safety and wellbeing.
  • Willingness to be on-call for emergencies.
  • Valid AZ driver’s license.
  • Reliable transportation and ability to show proof of registration/insurance.
  • Level One Fingerprint clearance or ability to obtain fingerprint clearance and maintain clearance.
  • Must be 21 years of age or older to drive on behalf of Catholic Charities.
  • Must have or complete within 90 days of hire, training in CPR and First Aid and remain current in the certification.

Responsibilities

  • Implement the Family Connections (FC) model and curriculum with fidelity.
  • Conduct home visits at a minimum once a week and deliver the FC Curriculum.
  • Prepare for home visits and complete documentation within one business day.
  • Conduct home visits at times convenient for the family, including evenings and weekends.
  • Conduct comprehensive family assessments and other required assessments.
  • Develop and implement Service Plans with family participation, ensuring UBSMART goals and interventions.
  • Maintain case records and data systems in accordance with agency and program policies.
  • Initiate contact and conduct a home visit with referred families within one business day of referral.
  • Attend mandated trainings, conferences, meetings, or community events, which may include out-of-state travel.
  • Connect families to resources that address their needs.
  • Maintain positive working relationships with colleagues, administration, collaborative partners, and community members.
  • Participate in one hour of individual supervision and one hour of group supervision per week.
  • Make appropriate referrals to government & community agencies and sub-contractors, and follow up on progress.
  • Report suspected child abuse or neglect to the Department of Child Safety (DCS).
  • Complete training in the Nurturing Parenting Program practice model.
  • Utilize or conduct intakes per the Nurturing Parenting model.
  • Collaborate with the parent/caregiver to complete the Family Nurturing Plan (FNP).
  • Conduct a minimum of two hour weekly in-person parenting sessions and document progress.
  • Schedule and conduct a mid-point session with the family to evaluate progress.
  • Close cases when targeted goals are addressed, client requests closure, service provision can no longer achieve goals, or identified reasons are met per the Nurturing Program Manual.
  • Complete documentation required for case closure.
  • Promote ongoing commitment to excellence and continual quality improvement.
  • Perform other duties as assigned.
