The Family Centered Clinician-In Home Therapist provides service and support to individuals and families, developing goal-oriented treatment plans based on a person-centered assessment of strengths and needs. This role involves servicing clients in Wakefield and surrounding areas, with visits scheduled Monday-Friday, including after school hours. The clinician will conduct person-centered assessments in various areas such as behavioral health, education, personality, community engagement, vocational needs, relationships, and family interactions. They will develop service or treatment plans in collaboration with referral agencies and the individuals served, and provide activity and participation-based interventions. The position requires 24/7 coverage for assigned individuals/families, making referrals to services and resources, and ensuring confidential, organized, and timely documentation. The clinician will also participate in team meetings, support peer development, provide aftercare plans, and build relationships with community partners. Additionally, this role may involve training or field supervision of peers, reviewing documentation, and conducting marketing and education presentations.
