About The Position

This position will serve as the family care navigator for the Division of Substance Use Prevention and Harm Reduction’s (SUPHR) Philly LIFTS (Linking Infants and Families to Service) program. Philly LIFTs is an outreach program for the families of infants diagnosed with Neonatal Abstinence Syndrome (NAS), along with families affected by substance use disorder at all stages. Philly LIFTs is part of SUPHR’s NAS Program, which conducts surveillance of NAS diagnoses in Philadelphia. The primary goal of the program is to support the families of infants diagnosed with NAS, along with families affected by substance use disorder. The program aims to “lift” parents and their children to support and services. This support includes, but is not limited to food referrals, baby supplies like diapers and car seats, clothing, employment assistance, referral to home visiting programs and doula support, recovery support, short-term therapy sessions for children 5-21 years old, mental health support for postpartum parents, and overdose prevention training. Philly LIFTs hosts community outreach events and celebrations throughout the year. These events are open to all families, not only those enrolled in the program. The Family Care Navigator will work in-tandem with the Senior Family Care Navigator to connect families impacted by substance use to needed clinical and social services. This individual will be responsible for supporting the continuity of care for families served by the program and will be situated within the Philadelphia Department of Public Health.
