About The Position

The Family Care Coordinator (FCC) provides intensive family support services to families of infants and young children who have or are at risk of developing disabilities or developmental delays. The FCC provides each family with a comprehensive orientation to early intervention and the Family Support Planning process. This role requires organization, companssion, and flexibility regarding schedule. We see clients during day light hours but weekends may be involved.

Requirements

Bachelor degree from an accredited college or university with major emphasis in early childhood, social work, special education, child development, health education, psychology, and/or interdisciplinary sociology. Bachelor degree in another area, to be considered for this position, must have additional experience (see below).
Must complete service coordination apprenticeship and enroll as a service coordinator with the Children Medical Services Early Steps State Office and with Medicaid Targeted Case Management and within 90 days of hire.
Valid Florida driver’s license with no more than six (6) points, at least 21 years of age, and must be insurable under the Orlando Health insurance carrier’s requirements and standards.
One (1) year of experience working with young children and early intervention is required.
If bachelor degree in another area of study than what is listed above - must have three year’s experience working with young children and early intervention.

Responsibilities

Establishes rapport and a continuous relationship with each family during the child’s first 3 years of life, providing support and advocacy as the family works on their Individualized Family Support Plan (IFSP).
Regularly meets with families in person, by phone or via telehealth platforms to orient them to the Early Steps program, provide screening assessments, schedule service provision, update the child’s IFSP and provide information and referrals.
Based upon the wishes of the family and in collaboration with them, other Developmental Center team members and involved agencies, works to meet family needs, such as the need for therapies, special equipment, counseling, information on parenting or prematurity, or assistance in locating other community resources.
Acts as a liaison between the family and multiple agencies, and helps families make informed choices about services for themselves and their children with special needs.
Ensures all activities are appropriately documented in case files, required data is gathered, and billing procedures are completed to support Targeted Case Management as required by the Florida Department of Health Early Steps State Office (ESSO) and Medicaid Program Guidelines. Maintains confidentiality of data and information in compliance with HIPAA regulations and Orlando Health policies.
Performs intake coordination services as needed. Receives and processes all incoming referral phone calls, faxes and emails. Describes program services and eligibility requirements to caregivers and referral sources. Gathers all pertinent information from caregivers to complete intake paperwork. Knowledgeable of all referral criteria, eligibility codes and insurance requirements. Provides guidance to caregivers and create case files.
Researches child’s information in the program database for prior program involvement. Researches child’s information in the Medicaid system to check on and verify Medicaid status and coverage, and completes and tracks appropriate correspondence. Creates child’s electronic and paper record within 24 hours.
Schedules and participates in multi-disciplinary team evaluations of children to determine eligibility for the Early Steps program.
Schedules and participates in transition meetings with local schools when children turn three years old.
Serves as the primary point of contact for caregivers of children for the duration of time they are in the Early Steps program, which may be up to 36 months.
Reviews each child’s IFSP every six months to assess progress in achieving goals and provide feedback to caregivers.
Schedules and participates in annual IFSP updates for each child on their caseload.
Supports and encourages parents/caregivers to become active participants in their child’s IFSP and development.
Maintains reasonably regular, punctual attendance consistent with Orlando Health policies, the ADAAA, FMLA and other federal, state and local standards.
Maintains compliance with all Orlando Health policies and procedures.
