Under general supervision, the Family Care Coordinator is responsible for coordinating care between medical, substance abuse, and mental health services. This position is subject to working evenings and weekends. No smoking workplace. Essential Duties and Responsibilities include the following. Other duties may be assigned. Helps families to navigate the largest health systems. Assists in SBIRT Services and FASD assessment. Provides MI for health-related issues. Provides outreach and engagement. Participates in scheduled training and meetings, as necessary. Participates in program data collection and submission as necessary to meet project goals. Maintains records and completes appropriate documentation.
