Job Description KVC is seeking a Family Care Coordinator / Case Manager to work directly with families on a day-to-day basis while also managing and coordinating client care. The Family Care Coordinator / Case Manager will work with the Family Preservation and Reunification Services contracted by the state.

Requirements

Bachelors in Social Work (or other related field)
Valid Driver's License and Car Insurance

Responsibilities

Establish initial contact with families within 24 hours of referral to assess safety
Initiate safety planning, and connect families with resources to meet immediate needs
Conduct team meetings and maintaining communication with the family and providers
Facilitating the implementation of each family’s Action Plan
Make frequent and consistent contact with DCBS personnel
Provide direct services and education targeted at reducing risks, improving child well-being, and preserving child placement with the family per program guidelines.
