Requirements

  • High school diploma or equivalent and 3 years' experience in early childhood or home visitation.
  • Possess an insured & reliable vehicle and a valid AZ driver’s license.
  • Valid Arizona Fingerprint Clearance Card - OR - must qualify for a valid Arizona Level One Fingerprint Clearance Card
  • DES Criminal Affidavit Required
  • AZ DHS Criminal History Affidavit Required
  • FPC DPS Verification Required

Nice To Haves

  • Associates degree or higher in human services or related area and 1 year of relevant work experience preferred.

Responsibilities

  • Conducts screenings and assessments for pregnant women and families with newborns to support healthy child development, strengthen positive parent–child relationships, and prevent child abuse and neglect.
  • Collaborates closely with Healthy Families Central Administration through the Department of Child Safety.
  • Interacts with families in various settings including hospitals, public places, and families’ homes while also maintaining confidentiality.
  • Manages referral and outreach, screen, and conduct assessments of families within allotted timeframes according to policy, program requirements, and accreditation standards.
  • Utilizes approved assessment tools to assist in evaluation of family needs.
  • Composes accurate and complete screening and enrollment assessments, containing all areas of exploration, including caregiver strengths and areas of concern.
  • Composes accurate intake forms assuring all data points are complete per contract and funder requirements.
  • Submits all documentation within the required HFAz and Maricopa County Site Specific policy timeframes from assessment.
  • Provides appropriate community-based resources to families on an as needed basis.
  • Develops and maintains positive working relationships with community partners to coordinate needs for referrals or collaborate services for families.
  • Utilizes required forms and database systems to accurately document home referrals, interventions, assessments, or other information needed as required by HFAZ contracts and funders.
  • Adheres to program requirements for the position held in the program.
