About The Position

In this position, the Family Resource Specialist acts as a Family Advocate and works with parents/caregivers and members of the family, who have cross system complex service needs, to provide quality advocacy, effective communication and collaboration between individuals and their service providers and connect them to additional community-based services as needed.

Requirements

Bachelors Degree in related field.
Experience with the population served.

Responsibilities

Maintain consultative working relationships with countywide front-line staff, supervisors, and families.
Demonstrate strong advocacy and care coordination skills that increase family and youth voice in decision making.
Knowledge of community-based services in Dutchess County to increase family knowledge, access, and participation in community-based services.
Maintain quality contact with families to successfully navigate multiple systems and providers on the individualized family plans by collaborating to implement, monitor, remove barriers and evaluate.
Accompany parents/caregivers and individuals to community meetings when requested.
Participate in community education and events to expand knowledge of community-based services.
Represents the Agency and families' interests within county and statewide meetings.
Participates in individual and team meetings, maintains records in full compliance with Agency regulations.
