About The Position

The Center for Family Development (Columbia, TN office location) is seeking a case manager to work with children that are being raised by relative caregivers that have custody of child(ren), to include: grandchildren, nieces, nephews, siblings, etc. This position will work closely with caregivers and children within local counties, within their homes, to empower families with the tools, resources, and support needed to keep children out of the foster care system, build resilience, and overcome barriers to care.

This position will provide guidance to facilitate the following core services:
Monthly in-home visits with all of the eligible children within the family home
Quarterly caregiver support groups
Family advocacy and crisis intervention
Information about applying for TANF, SNAP, Child Only Grant
Support in locating birth certificates, social security cards, and insurance cards
Emergent caregiver financial assistance

Requirements

• Must have reliable transportation
• Ability to commute to Columbia, TN 38401
• Ability to relocate to Columbia, TN 38401 before starting work

Nice To Haves

• Bachelor's degree (Preferred)
• 2 years case management experience (Preferred)

Responsibilities
