About The Position

Boley Centers, Inc. is seeking a Florida Assertive Community Treatment (FACT) Team Leader/Supervisor to establish, administer, and direct a self-contained clinical team. This team provides treatment, rehabilitation, and support services to clients with severe and persistent mental illnesses. The role involves supervising and evaluating a multidisciplinary team, ensuring service excellence, and functioning as a practicing clinician. As the program operates 24/7, the Team Leader/Supervisor may be required to work outside standard business hours, including weekends and holidays. This is an in-person, onsite position.

Requirements

  • Master's Degree in social work or mental health counseling.
  • LCSW/LMHC required.
  • Registered counseling intern considered if within six months of licensure and licensure exam passed.
  • Education must be from an accredited school, college, or university.
  • Must be licensed or license eligible.
  • Minimum of five years relevant experience.
  • Significant experience in administration, case management, and staff supervision.
  • Valid Florida driver's license.
  • Own transportation vehicle and insurance required.
  • Must pass a Level II background check and drug screen.

Responsibilities

  • Directs the day-to-day clinical operations of the team, including scheduling staff for day, evening, weekend, holiday shifts, and on-call hours.
  • Leads daily staff meetings and treatment planning meetings.
  • Continuously evaluates client status and coordinates treatment activities to address changing needs.
  • Directs and coordinates the client admission process and treatment, rehabilitation, and support services in conjunction with the psychiatrist.
  • Schedules admission interviews, develops and coordinates initial assessments and treatment plans, and assigns appropriate staff to treatment teams.
  • Provides clinical supervision for the development of comprehensive assessments and treatment plans.
  • Directs and coordinates comprehensive assessments for each client, covering psychiatric history, mental status, diagnosis, medical/dental health, substance use, education/employment, social functioning, daily living activities, and family relationships.
  • Manages all aspects of day-to-day FACT staff management, including hiring, work assignments, and performance management.
  • Manages Consumer Contingency funds.
  • Operates personal or agency vehicle to transport clients and/or travel between locations.
  • Assists the Director in developing and documenting performance measures and reporting results to the Q.I. Committee.
  • Assists the Director with corrective action plans as needed.
  • Supervises medical records management, ensuring compliance with agency policies, Medicaid, and other third-party requirements.
  • Trains staff on medical record requirements and regularly reviews client assessments, treatment plans, and progress notes.
  • Supervises individual staff for medical records mastery.
  • Initiates and maintains relationships with law enforcement, other human service agencies, hospitals, and community resources.
  • Conducts clinical assessments and provides direct clinical treatment, rehabilitation, and support services to clients.
  • Performs on-call duty for crisis intervention and for staff supervision and consultation.
