About The Position

The Facility Rehab Lead serves as the on-site representative of UT Health at Home within partnered senior living and congregate care communities. This role supports the coordination of clinical and non-clinical wellness services to enhance resident safety, engagement, and overall quality of life. The Rehab Lead works closely with residents, families, therapy teams, nursing staff, and community partners to ensure a seamless home health experience and exceptional customer service.

Requirements

  • High School Diploma or equivalent required
  • Six months to one year of related healthcare, senior living, wellness, or customer service experience required
  • Knowledge of medical terminology preferred
  • Strong interpersonal, communication, and organizational skills
  • Ability to work collaboratively with interdisciplinary teams
  • Reliable transportation with valid driver’s license and current auto liability insurance required

Responsibilities

  • Coordinate and assist with scheduling skilled nursing and therapy visits for residents
  • Participate in daily stand-up meetings to communicate updates regarding home health patients
  • Report changes in resident condition to appropriate clinical staff in a timely manner
  • Lead and oversee non-clinical wellness programs and resident engagement activities, including: Instructor-led exercise classes, Supervision of scheduled gym hours, Wellness workshops and educational events, Assistive device clinics and balance screenings, Dementia care programming
  • Provide outstanding customer service to residents, families, and community partners
  • Participate in prospective resident tours and provide clinical-related guidance regarding available services
  • Educate residents and families on appropriate levels of care based on individual needs
  • Assist residents and families with the home health onboarding process during or prior to move-in
  • Conduct move-in safety assessments to support a safe and successful transition into the community
  • Maintain wellness equipment and furniture, including basic assembly and disassembly when needed
  • Promote company culture, mission, and values through professional communication, appearance, and service excellence
