About The Position

This role is responsible for completing screening tools to identify and minimize extended lengths of stay for patients. The position promotes the Home First Philosophy and involves collecting and analyzing data to determine outcomes. The Extended Length of Stay (ELOS) Risk Assessor will work under the general direction of the Manager, Utilization, Care Transitions, & ALC Leading Practices.

Requirements

Minimum of a Bachelor’s Degree in a health-related field or equivalent.
Ministry of Labour “Worker Health and Safety Awareness in 4 Steps” training certificate is required.
Minimum of two (2) years’ experience working within a health care environment.

Demonstrated excellent presentation skills.
Demonstrated knowledge of Medical Terminology.

Nice To Haves

Training in Lean methodologies is preferred.
Experience working with or knowledge of assessment tools (i.e. InterRAI, Barthel Index, Blaylock, etc.) is preferred.
Experience working with the elderly and their care givers/informal supporters is preferred.

Responsibilities

Engage patients in initial Home First Philosophy conversation.
Meet with patients to complete screening tools used to assess risk for an extended length of stay.
Determine priority of focus to achieve an estimated date of discharge (i.e., clinical, social, functional).
In collaboration with a regulated health professional, review completed screening tool(s), assess results and make recommendations on service required based on score.
Act as a point of contact for departmental staff, other departments, programs, and community stakeholders as required.
Attend daily meetings (e.g., Emergency department huddles, bed rounds, bullet rounds, etc.) and be aware of daily bed pressures.
Communicate across care team and engage with community partners effectively.
Develop strong relationships with physicians and interdisciplinary team members in the Emergency department (ED) and across the organization.
Document score and action taken/required on appropriate form and attach to the patient chart.
Provide support for patients, caregivers and informal supporters being served by the organization and community agencies.
Determine and align improvement projects with HSN’s Strategic Plan; monitor and adjust to achieve goal outcomes.
Contribute to learner development by providing academic support, mentorship, preceptorship and supervision as required.
Participate in education and training specific to current, relevant federal and provincial health and safety legislation, standards and guidelines.
Educate and promote health, safety and wellness in the work place.
Represent the department or program on various committees and in meetings as required.
Perform other duties as required.
