Program Overview: In partnership with Arizona's Department of Children's Services (DCS), Youth Villages provides support to young people (ages 17 to 23) wanting to maintain eligibility for extended foster care services through the Extended Foster Care (EFC) Program. The goal of EFC is to ensure better outcomes for young people as they transition into adulthood. The program offers individualized, clinically focused case management and counseling support across key areas: interpersonal connectedness/social support, housing, mental and physical health, career and employment, life skills, and education. EFC equips young people with the self-sufficiency and self-advocacy skills needed for long-term success after foster care. Position Overview: The EFC Success Coach engages youth in a coach-like way and provides strengths-based case management and interventions as needed for youth and young adults preparing for and participating in the Extended Foster Care Program.
