As a Transitions Hospice Night Nurse, you'll serve as a critical support member of the interdisciplinary care team during overnight hours — assessing, treating, and communicating patient needs with clinical confidence and genuine compassion.

Requirements

  • Active RN or LPN license in the state of employment
  • Graduate of an accredited school of Nursing.
  • Clinical experience in hospice, home health, or a related setting preferred
  • Comfort and composure working independently during overnight hours
  • Strong assessment skills and sound clinical judgment in high-pressure moments
  • A calm, reassuring presence for patients and families during nighttime distress
  • Excellent documentation habits and communication skills
  • Valid driver's license and willingness to travel locally

Responsibilities

  • Delivering professional nursing care across the full nursing process within your scope of practice
  • Conducting thorough patient and family assessments and contributing to individualized care plans
  • Completing all visit types as needed to meet overnight patient and family needs
  • Administering medications and treatments per physician orders
  • Identifying nursing diagnoses from health assessment data and responding with sound clinical judgment
  • Recording accurate, timely clinical notes including pain and symptom management outcomes
  • Communicating changes in patient condition to physicians and obtaining orders as required
  • Coordinating with community health partners to support continuity of care
  • Ensuring equipment and necessary services are in place for patients
  • Maintaining and growing your clinical skills through continuing education and in-service participation
