About The Position

US Career Partners is recruiting an experienced Nurse Practitioner or Physician Assistant for a well-established community outpatient medical center delivering palliative care across New Jersey. The role provides outpatient and skilled nursing facility (SNF)-based palliative care for patients with serious illness, working across SNF and community settings in Essex and Hudson counties. The posting also identifies the setting as outpatient in Burlington County, New Jersey, and notes that local travel within Burlington County is required, with mileage reimbursement and clinical/EMR support provided. The clinician will deliver holistic, patient-centered, evidence-based interdisciplinary palliative medicine; conduct consultations, assessments, and follow-up visits; manage complex symptoms; lead goals-of-care and advance care planning conversations; and coordinate with patients, families, facility staff, primary care, hospice, and home-health partners. The role includes supporting care transitions from hospital admission through SNF stay, discharge, rehospitalization decisions, hospice transition, or other next-site-of-care planning, as well as completing timely documentation aligned with clinical, billing, and quality standards. The position offers a structured ramp into palliative care, mentorship, clinical backup, data-guided coaching, clear workflows, and support from physicians, APPs, social work, care coordination, and operations. The primarily Monday–Friday schedule offers flexibility to swap workdays. Compensation is a base salary of $130,000–$160,000 per year plus an annual performance bonus of up to 12.5% of base salary. The role is full-time, though part-time or per-diem arrangements may be discussed for the right candidate.
