About The Position
Responsible for the day-to-day implementation and follow-up case management for patients in Cardiac Rehab. Provides individualized rehab programs to meet the personal and family needs of patients. This position is responsible for outpatient classes and requires an understanding of cardiovascular disease.
Requirements
- Bachelor’s degree in EP or related exercise science field
- BLS and ACLS certification
Responsibilities
- Performs day to day implementation and follow up case management for patients in Cardiac Rehab.
- Provides individualized rehab program to meet personal and family needs of patients.
- Responsible for outpatient classes.
- Assesses patients physical, psychosocial, educational needs on admission.
- Individualizes patients plan of care.
- Performs no invasive cardiology tests.
- Documents patient care assessments.
- Performs related duties as required.
