About The Position
This role provides comprehensive clinical case management services, including assessments, individual, group, and family treatment, treatment plan coordination, referent relations, discharge planning, and intervention with client support systems. The Evening Manager will also assist the Program Director with various oversight and management duties.
Requirements
- New York State CASAC and/or Licensed Master of Social Work
Responsibilities
- Coordination of census.
- Coordination of patient transfers.
- Overseeing the caseloads and scheduling of the Adult Program Clinical Case Managers and attend team meetings and audit paperwork as needed.
- Overseeing treatment planning.
- Evaluation and delegation of rules for the Program as put forth by management.
- Addressing all conflict issues with both staff and patients.
- Conducting wing meetings.
- Reviewing and modifying the program’s services.
- Public relations with other treatment modalities to assess appropriate referral for adult clientele.
- Maintaining Coordinator’s own reduced case load and responsibilities as outlined in Case Manager (CASAC/LMSW) job description relative to licensure and title.
- Participates in Performance Improvement Initiatives.
