ES Case Management Specialist
About The Position
Our ES Case Management Specialist works in our office from 9:00 am - 5:00 pm, Mondays, 10:00 am - 5:00 pm, Tuesdays, 1:00 pm - 5:00 pm, Wednesdays, and 10:00 am - 5:00 pm, Thursdays and Fridays. You'll provide attentive, individualized care to people in crisis, helping individuals, families, and couples enhance their well-being and access the support they need. Supporting mobile crisis-paired response initiatives, you'll provide compassionate case management services related to transportation, vocation, referrals, psychosocial education, and community engagement. You'll address each person's unique needs and deliver the highest quality of services possible. In addition to these services, you'll also help arrange counseling services for individuals, couples, groups, and families, and tackle additional responsibilities as needed.
Requirements
- Bachelor's degree
- Colorado Crisis Professional certification or the ability to obtain
- 3+ years of experience in the human services field
- Valid driver's license
