About The Position

This position is responsible for providing comprehensive pharmaceutical care to patients in the Emergency Room. The ER Clinical Pharmacist will collaborate with physicians and other healthcare professionals to optimize medication therapy, ensure patient safety, and improve health outcomes. This role involves direct patient care, medication reconciliation, drug information services, and participation in interdisciplinary rounds.

Requirements

  • Current license as a Pharmacist through the West Virginia State Board of Pharmacy.
  • Current license as a Pharmacist through the Pennsylvania State Board of Pharmacy.
  • Obtain certification in Basic Life Support within 60 days of hire date.
  • Pass a state criminal background check and Federal background check (if applicable) for regulated areas.

Nice To Haves

  • Doctor of Pharmacy (PharmD) from an accredited school of pharmacy.
  • Completion of a residency training program.
  • Three (3) years of pharmacist experience.

Responsibilities

  • Provide direct patient care in the Emergency Room setting.
  • Collaborate with physicians and healthcare team members to optimize medication therapy.
  • Ensure patient safety through medication reconciliation and adverse drug event monitoring.
  • Provide drug information services to healthcare professionals and patients.
  • Participate in interdisciplinary rounds and patient care conferences.
  • Maintain accurate and complete patient records.
  • Adhere to all hospital policies and procedures, as well as state and federal regulations.
