About The Position

Project Renewal is a leading non-profit organization with the mission of building supportive communities where people achieve dignity and independence, renewing their lives with health, homes, and jobs. Our comprehensive and integrated 24/7 programs are designed to offer the holistic care our clients need. With a staff of 950+ and an annual budget of $140 million, Project Renewal is one of the larger social service nonprofits in New York City. The Barbara Kleiman Men’s Shelter is a 200-bed, 24/7 emergency shelter for 200 men who are experiencing homelessness. The program will serve individuals diagnosed with mental illness and/or substance use disorders. Our comprehensive services include case management, individual and group counseling, mental health care and primary medical care, recreational activities, occupational therapy, employment services and job readiness services, and housing placement assistance.

Requirements

  • High School Diploma or GED required
  • A minimum of one (2) years of experience working with in a non-profit or government organization providing social services to a target population which includes but is not limited to the homeless, mentally ill, substance, use, HIV/AIDS, criminal justice, and LGBTQIA+
  • Knowledge of Case Management software (CARES)
  • Proficiency in Microsoft Office Suite, especially Outlook, Word, and Excel.

  • Ability to work cooperatively with peers and other staff in order to serve the needs of the clients as effectively and efficiently as possible
  • An ability to work independently and to adjust one’s schedule to accommodate the needs of the clients
  • Excellent oral, writing, and listening skills
  • Familiarity with eClinicalWorks or other medical software a plus
  • Bilingual or multilingual a plus

Responsibilities

  • Assists clients with access to available public benefits including but not limited to Public Assistance, SNAP, public health insurance, and Federal disability benefits
  • Makes eligibility determinations, processes applications, and completes all related follow-up
  • Develops resources and contacts to facilitate clients’ access to benefits in a timely manner
  • Complete case notes for all interactions with clients and for collateral contacts with outside agencies
  • Escort clients to Fair Hearings and other HRA appointments, as necessary
  • Works in conjunction with the Case Managers, other clinical department staff members to schedule/accompany clients to appointments for entitlements and healthcare
  • Coordinates reminders and transportation arrangements to ensure that appointments are kept
  • Provides coaching to clients about how to maximize and keep their benefits active
  • Provides clients and staff with updated information and direction in acquiring and maintaining entitlements with city, state, and federal entitlement programs
  • Assist clients in completing medical registration packets and creating initial charts in eCW
  • Assist in scheduling clients for initial and follow-up appointments with onsite psychiatric provider
  • Performs other duties as assigned by the Program Director and or Director of Social Services
