About The Position

The Enhanced Care Coordinator provides case management, skills training, and coordination of services to individuals with mental illness within the community and outside of the local area. Provision of treatment services are informed by the Individual Service and Support Plan (ISSP) and clinically supervised by the primary clinician. This classification provides advocacy and linkage in planning, monitoring, coordination, and implementation of the treatment interventions as identified in the ISSP. The ENCC must be able to communicate well with statewide resources in coordination of the care of individuals leaving the areas as well as transitioning back into the area, assisting with problem-solving to reduce barriers to recovery, and supporting the development of resources within the community while coordinating services with other community and treatment providers. This position ensures accurate tracking of funds, authorization of services, and other required documentation for Choice Model and Adult Foster care. This classification coordinates and collaborates with a multi-disciplinary treatment team and is responsible for work performed in assigned specialized program area. The ENCC also will license and monitor Adult foster home providers for regulation with Oregon Administrative Rule providing direction and guidance.

Requirements

  • Bachelor’s degree in psychology, social work science or related field, plus two years of experience in a mental health setting OR a satisfactory equivalence of education, experience, and training
  • Must be 21+ and possess a valid driver’s license

Responsibilities

  • Provides case management, coordination and crisis intervention as directed by a QMHP and documented in the Individual Service and Support Plan (ISSP); uses interventions informed by Evidence-based Practices endorsed by Compass Behavioral Health, Enhanced Care Coordination duties assisting members in facilitation of placement and services throughout the state.
  • Actively promotes the harmonious delivery of services through coordination of interventions in collaboration with treatment team members and foster providers achieved by ongoing communication and active problem solving with the treatment team and foster providers about outcomes, barriers and case observations.
  • Maintains and completes required records, reports, documentation as required by departmental policy and state and federal law.
  • As many of the services are provided with in the community and throughout Douglas County, driving is an essential and daily function of this position; transports individuals, family members and other supports to/from services.
  • Emergency Response: This employee has been identified as a staff member expected to report for duty following activation of a public health emergency response plan.
