About The Position

We care deeply about the individuals and families within our community, and it is our priority to ensure timely access to services and support. The Engagement Specialist is responsible for contacting clients who miss scheduled service appointments to confirm their interest in continuing services. This role works collaboratively with clients to identify and address barriers that may prevent attendance at scheduled sessions, support re-engagement in treatment, and adjust the level of care when appropriate. The Engagement Specialist also initiates discharge and transfer planning for clients who cannot be successfully re-engaged in treatment services. Additionally, after repeated late cancellations or no-show appointments, this position provides referrals and links clients to other community and human service programs to support ongoing care needs. As part of the care team, the Engagement Specialist shares responsibility for directing, coordinating, and managing client care and services to promote positive outcomes and continuity of care.

Requirements

  • Bachelor’s Degree or equivalent experience
  • Skilled in operating various medical record software and hardware, word-processing, and database software programs
  • Spanish Speaking required
  • Excellent multitasking and communication skills
  • Ability to work independently and part of a team
  • Current driver’s license, reliable transportation, registration and auto insurance
  • Ability to communicate effectively
  • Travel to and from clients’ residence, community locations and office site, which could include using walkways, stairs and/or elevators
  • Ability to lift up to 20lbs
  • Must be able to work remotely and in person adhering to PHI requirements

Nice To Haves

  • Behavioral Health experience preferred
  • Bilingual/ASL skills

Responsibilities
