About The Position

Senscio Systems is a Harvard, MA based company with current operations in MA, NH ME, RI, CT, PA, NC and FL. Our passion is leveraging a unique, patented AI technology with a collaborative multi-disciplinary clinical team to support patients (we call them Members) with complex medical challenges. As a key member of the Senscio team, you will be in the central role of providing real-time self-management support to our Members to enable them to continue to live comfortably and safely in their homes and communities. Senscio’s 24/7 home-based virtual program acts as an extension of our Member’s primary care team, providing their PCP and associated specialists w/actionable situational awareness. Integrating physical and behavioral health support with a broad scope of social determinants of health needs, we provide a truly holistic, patient- centered plan of self-management support. We have shown dramatic improvement in clinical outcomes and overwhelmingly positive Member experience scores, leading to tremendous projected growth in membership.

Requirements

  • Must be physically located in the New England area; preferably Maine, New Hampshire, Massachusetts, Rhode Island or Connecticut.
  • Certification as a Medical Assistant, CNA, Community Health Worker or some background in Behavioral Health strongly preferred.
  • Energetic, self-starter with a demonstrated work ethic.
  • Computer/technology savvy.
  • High level of energy and capable of/willing to work in a fast moving, rapidly changing environment.
  • Strong communication skills/willingness to build trusted relationships quickly.
  • Ability to work independently and remotely, but as part of a highly integrated and collaborative team.
  • Past remote work record of success highly valued.
  • Passion and strong empathy to support a challenging and vulnerable population in achieving some level of self-reliance and independence.

Nice To Haves

  • Degree in Psychology or some area of social work a plus.
  • Chronic disease management, to include Behavioral Health and Social Determinants of Health knowledge a plus.

Responsibilities

  • Providing real-time self-management support to Members to enable them to continue to live comfortably and safely in their homes and communities.
  • Reviewing tasks, issues, or requests from or about a panel of Members through the technology platform.
  • Observing Members' success and progress in completing tasks assigned through their personalized care plan and coaching as necessary.
  • Engaging directly with Members to assist them if they are outside the realm of routine tasks and coaching (e.g., break in device engagement, failing to take vital signs, missed medications, technology challenges).
  • Escalating information to the next level of the care team for triage and care when necessary.
  • Coordinating with local community-based organizations to assist Members with social challenges such as food insecurity or transportation.
  • Developing strong, trusting and meaningful relationships through value-added support to Members.
