About The Position

The Encounter Specialist will participate in daily Care Team Huddles to ensure collaborative care is given to each patient on every visit. They will perform a review of the patient's medical records for upcoming, scheduled clinic visits, abstracting pertinent data related to activity within the healthcare system since the last visit, quality indicators, and the patient's personal clinical goals of therapy. The specialist will make notes as necessary to serve as a reminder of issues that need to be addressed during the clinic visit. They will greet and escort patients to the exam room, obtain and document all pertinent vital signs and measurements, and assess the medication regimen in detail to evaluate each medication and dosage currently in the EMR and any newly reported medications or dosages, noting any discrepancies to discuss with the provider. The role also involves reconciling allergies, engaging the patient in conversation regarding the reason for visit and overall health, assessing for social environment, gaps in care, quality measures, and progression and/or barriers to reaching goals of therapy. The specialist will prepare charts with any labs or other data for ease of access for the provider, including reminders for quality indicators. They will remain in the exam room, enter all findings, including physical exam and assessment provider notes, enter provider orders and plan of care instructions, including prescriptions, and provide instructions to the patient and educational materials based on the exam findings. Additionally, the Encounter Specialist will document all activities/interventions, patient/family responses, and medication dispensed/prescribed in the electronic medical record. They will also fulfill other patient care responsibilities as assigned, which may include checking schedules and organizing patient flow; assisting patients as needed with walking, transfers, dressing, collecting specimens, preparing for exam; and answering calls. Clerical responsibilities may include sending/receiving medical records and reports; facilitation of referrals; completing forms and requisitions; scheduling appointments; insurance verification and pre-certification activities. The role also involves enhancing professional growth and development through participation in in-service meetings, educational programs, conferences, and committees as requested, and adhering to applicable states controlled substance regulations. The specialist will practice universal precautions and properly handle blood and body fluids and deal with infectious patients. Finally, they will complete training related to hierarchical condition category (HCC) coding and demonstrate competency by appropriately transcribing patient visits.

Requirements

• Basic Life Support (BLS) for Healthcare Providers certification issued by the American Heart Association (AHA)
• CMA from American Association of Medical Assistants (AAMA) OR CCMA from National Health career Association (NHA) OR NCMA from National Center for Competency Testing (NCCT) OR RMA from American Medical Technologists (AMT) OR RMA from American Allied Health OR NCMSMA from National Healthcare Worker’s Association (NHCWA) OR NRCMA from National Association for Health Professionals (NAHP) OR NRMA from National Registry of Medical Assistants (NRMA)
• GED or HS Equivalency Diploma.
• Graduation from an accredited Medical Assistant program required
• One year of Scribe experience, preferred
• Previous experience in medical practice preferred.
• Knowledge of computer systems and applications
• Advanced typing and navigation skills within the electronic medical record system
• Critical thinking skills
• Solid oral and written communication skills
• Advanced knowledge of medical terminology, disease specific goals of treatment, and assessment skills
• Knowledge of medical documentation guidelines and practices including legal responsibilities related to confidentiality
• Knowledge of standard concepts, practices, and procedures within the designated field
• Knowledge of organizational policies, procedures and protocols
• Knowledge of common safety hazards and universal precautions to establish a safe work environment
