About The Position

Manage the ENCC program working with individuals identified by Outpatient services, Intensive services, SUD services, and GOBHI that need specific exceptional needs care coordination. Provide leadership and strong dedication to develop and deliver evidence-based services and support to individuals with mental illness and co-occurring disorders. Work closely with psychiatric hospitals in Oregon and Idaho regarding discharge planning for specific clients as Malheur County acute care coordinator including 3 day and 7 day follow ups. Work closely with GOBHI and provide reports on 3 and 7 day follow ups. Update, complete, and/or assign staff to complete daily pre-management report. Assist with obtaining and/or assign staff to complete eligibility referral documentation for helping clients in & out of residential services. Ensure client referrals are completed as needed. Assist with long-term care requests. Ensure member care plans are completed. Help provide adult mental health foster home licensing and oversight for Malheur County. Coordinate with team gathering necessary documents to support foster home placement, track resident placement, and resident progress in services. Provide documentation to foster homes to develop Residential Care Plans. Ensure all required program reports are completed timely. Participate in monthly operation reviews and have accountability to program budget. Develop and maintain relationships with community partners. Observe, supervise, and assist ENCC’s.

Requirements

  • Master’s degree in behavioral health or related field
  • Two years’ experience working within and/or education within the mental health field
  • Two years of leadership or supervisory experience
  • Hold an Oregon QMHP certification and ability to provide supervision.
  • Current driver's license
  • Successful completion of background check
  • Tuberculosis screening

Nice To Haves

  • Two or more years’ experience coordinating care for individuals who suffer from mental health and substance abuse symptoms
  • Strong knowledge of mental health and substance abuse systems of care and how to coordinate and work effectively within those same systems
