About The Position

Provides comprehensive care to residential patients; provides medication monitoring, education, and prescriber collaboration; provides information, education and support for clients; promotes wellness, recovery and skill building to sustaining better heath; prevention training in the areas of wellness, recovery, managing symptoms, making healthy life style choices, recognizing and responding to relapse, managing crisis and achieving and sustaining recovery. It is expected that the EMT function as an integral, cooperative member of the agency team of professionals and will provide outstanding internal and external customer service and maintenance of appropriate documentation.

Requirements

  • Staff utilizing electronic health records and/or other automated/electronic documentation systems are required to have computer and keyboarding experience and skills or the ability to develop such skills within required timeframes in order to meet the level of proficiency set by the agency.
  • All employees in the EMT position must successfully complete required background checks and pre-employment screening.
  • Certification as Emergency Medical Technician
  • Must have and maintain driver’s license and good driving record.
  • Effectively communicate via written, verbal, in person and virtual methods.
  • Knowledge of guidelines for confidentiality
  • Knowledge of applicable regulatory guidelines and codes, agency policies and procedures, and Federal and state laws pertaining to position – including SUPR rules
  • Knowledge of main effects, side effects and interaction effects of a broad range of psychotropic mediations.
  • Knowledge of both psychiatric and physiological causative bases for illness.

Responsibilities

  • Assists service recipient in developing an individualized treatment plan, must be person centered, family centered, and service centered.
  • Monitors service recipient progress in treatment and participate in treatment plan reviews as assigned.
  • Updates treatment plans as appropriate.
  • Submits treatment plans at all stages for medical review, if applicable.
  • Conducts intake for recipient entering services, including explaining services, gathering information needed on intake materials, ensuring service recipient understands all forms, assuring signature on appropriate contracts and consents, and submitting materials for processing, as assigned.
  • Conducts assessment and write assessment summaries, as assigned.
  • Assist service recipients with making linkage with internal and external resources as outlined in treatment plan and in coordination with Integrated Healthcare Services.
  • Maintains appropriate contact with referral sources including providing information on service recipient progress, behavior, and needs (with appropriate releases), and maintaining ongoing consultation when appropriate.
  • Assist service recipient in making discharge plans and provide follow up services as assigned.
  • Monitors self-medication protocols, monitors the dosage, side effects, use and documents patients’ reactions.
  • Provides medication training to the client or client’s family or guardian to administer the client’s medication, to monitor proper levels of dosage and watch for side effects.
  • Obtains laboratory specimens and help doctors with patient examination and continuity of care arrangements with other providers.
  • Monitors urinalysis, breathalyzer, and other testing as assigned.
  • Meets and maintains applicable certification and licensure requirements.
