About The Position

Gather Health is seeking an EMT or Paramedic to provide in-home care and support for older adult patients facing social challenges, referred to as Social Determinants of Health (SDOH), such as lack of transportation. This role, internally called "Medical Technician Partner" (MTP), will act as the eyes, ears, and hands of the medical teams in the patient's home. The MTP will collaborate with primary care providers, Gather Guide Partners, and clinic administrators within a care-team model. The position is crucial for delivering high-quality care and support, contributing to Gather Health's vision of creating equity in healthcare.

Requirements

  • Minimum of 1 year of experience working as an EMT or in direct patient care.
  • Active certification as an EMT-B, EMT-A, or Paramedic in Massachusetts.
  • Active certification in AHA or Red Cross Healthcare Provider CPR.
  • Active unrestricted US driver's license.
  • Ability to work for any employer in the United States.

Nice To Haves

  • Experience working with senior populations, particularly with complex social needs.
  • Experience with basic office tools such as email, Outlook calendar, etc.
  • Strong verbal, written, and reading comprehension skills in English.
  • Secondary language capabilities.
  • Active certification as a Phlebotomy Technician.

Responsibilities

  • Provide in-home care and support for senior patients.
  • Serve as a clinical liaison between patients and primary care providers.
  • Perform approximately six to eight home visits per day.
  • Address acute medical decompensation or provide routine clinical care, including phlebotomy and point-of-care testing (EKGs, UAs, INRs, respiratory swabs, etc.).
  • Check in and organize an inpatient or ED admission avoidance plan.
  • Conduct social visits with patients in their homes.
  • Make outbound calls to patients to check on their health status.
  • Utilize Electronic Health Record (Athena) to review patient appointments, charts, and document notes.
  • Leverage and access medical control through the Clinical Medical Director as needed to deliver care.
  • Perform additional duties as assigned.
