POSITION DESCRIPTION: This position is responsible for the Case Management of all group employment referrals and members. Responsible for ensuring that individuals served in the department obtain and maintain meaningful employment within the community. Perform all case management tasks including but not limited to completing progress summaries, assessments and goals, attending ISP meetings, completing job evaluations and creating career plans.

Requirements

  • Associate degree with a minimum of 1 year experience in a supported employment program or High School diploma/GED with 4 years’ experience working with developmentally disabled adults in a supported employment program or in a position of case management.
  • Ability to complete all initial and ongoing background checks including but not limited to: CORI, SORI, Fingerprinting, LEIE, driving record, E-verify, Physical, PPD and licensing and/or certification checks (i.e., nursing).
  • Valid US driver’s license and ready access to a vehicle.

  • Must maintain all certification needed in the program including CPR, First Aid, ), and BFAIR Human Rights Training.
  • May need to attend workshops, seminars or BFAIR functions as needed for job development.

Responsibilities

  • Treat each individual, co-worker, supervisor and others with the respect and dignity due every human being and adhere to the Department of Developmental Disabilities (DDS) Human Rights Standards and the Disabled Persons Protection commission’s (DPPC) Mandated Reporter Guidelines around physical and emotional abuse, including timely reporting violations.
  • Ensure each individual may achieve optimal quality of life (QOL) outcomes for people we support through a person-centered approach utilizing universal strategies, which is the foundation of the quality care and service provided.
  • Ensure program meets quality assurance and certification standards by monthly review of necessary documentation and routine participation in and/or observation of services provided.
  • Work with community business to develop employment opportunities for people served and for vocational evaluation purposes.
  • Work individually with people as indicated, including providing physical assistance and/or supervision with mobility (light and heavy lifting and transfer), assist with hygiene, task training, recreational activities, or other needs.
  • Provide coverage at sites as needed in accordance with the contents of each individual’s ISP, medication treatment plan and documented behavioral interventions.
  • Manage referral and intake process in conjunction with Q&A and funding sources.
  • Develop and maintain positive working relationships with other funding sources by working collaboratively ensuring open and consistent communication.
  • Oversee assigned work sites: Provide a level of support which meets the needs of the individual’s site
  • Provide task training as needed
  • Ensure participants job expectations/description is clear; clarify any necessary modifications
  • Collaborate with the Director on vocational evaluation, assessment intake, and referral process. Including completion of all DDS vocational assessment reports as assigned.
  • Complete all facets of ISP process: Complete annual ISP assessments for review by Director
  • Attend ISP meetings for all people on caseload
  • Participate in the team process of developing plans focused on the unique needs and goals of the person served
  • Collaborate with DDS to ensure that ISPs are followed consistently
  • Oversee goal and objective development
  • Develop and update implementation strategies, teaching plan & PBS universal support
  • Utilize data collection tools as needed per PBS
  • Review evaluation procedures and assessment information to reflect changes made at staff meetings, ISP modifications, clinical team meetings and behavioral consultations.
  • Complete other ISP documentation to include: Individual employment evaluation, Job Exploration, Career Plans, Assistive Technology assessments, Other items that are required for employment
  • Update/Maintain Individual Records as needed.
  • Responsible for timely submission of billing to DDS/MassAbility.
  • Participate in clinical team meetings as needed for consultation purposes as well as coordination of services across all settings.
  • Meet weekly with Director; attend staff meetings and other meetings as needed.
  • Participate in Office of Quality Enhancement (OQE) preparation to ensure the department meets all standards to maintain licensure and certification.
  • Ensure implementation of all agency policies and procedures as specified in agency personnel policies handbook and program policies and procedures manual.
  • Act as an appropriate role model through dress, speech, and actions.
  • Ensure all regulatory standards are met including staff ratios, safety plans, fire drills, tests of emergency plans, semiannual site inspections, and equipment monitoring.
  • Manage crisis situation in conjunction with the management team and follow emergency procedures.
  • Participate in mentoring/delegation of tasks to Senior Employment Coach(es).
  • Assume responsibilities of Director of Employment in their absence.
  • Responsible for rotating on-call duties.
  • Represent BFAIR As requested at meetings, networking or community events.
  • Assure timely reporting and complete documentation of significant incidents, etc. in accordance with DDS regulations.
  • Arrange all staff PTO time off requests with Assistant Director Director. Vacation time must be requested at least two weeks prior to the desired date(s).
  • In the case of illness, you must call out to your supervisor two hours prior to the start of your shift.
  • Accurately documents all hours worked on the Agency electronic time management system. All timesheets must be submitted timely for approval which act as your electronic signature and approval. Time sheets should reflect employee’s assigned scheduled unless prior approval was given by supervisor to alter assigned schedule.
  • Responsible for maintaining current certification(s) including but not limited to START, CPR/FA, and MAP (if applicable to position).
  • Responsible for completing all training assigned in electronic Learning Management System on time or attending any scheduled training. If unable to complete or attend it is the employee’s responsibility to obtain prior authorization/permission to re-schedule training from Supervisor and notify Human Resources.
  • Responsible for routinely checking electronic communications; including but not limited to time off management, BFAIR email, BFAIR portal, and electronic LMS system. These systems should be checked each shift your work.
  • Serve as a member and take an active role in various committee assignments and responsibilities
  • Conduct training as assigned for New Hire Orientation and Staff Development.
  • Perform other duties as assigned by Director or designee.
