About The Position

The Employee Evaluation Specialist is a highly self-disciplined, motivated professional who helps protect the quality of care delivered in every patient’s home. This role conducts timely, meaningful caregiver evaluations; verifies that performance, documentation, and conduct meet Impact Home Health standards; and creates structured opportunities for feedback, coaching, recognition, and growth. Working closely with Advocate Caregivers, Human Resources, Staffing, Clinical, and Operations teams, the Specialist promotes employee engagement, accountability, compliance, and consistent performance expectations. The ideal candidate is detail-oriented, organized, empathetic, and confident in holding a high bar with humility while helping caregivers succeed.

Requirements

• High school diploma or GED required; associate or bachelor’s degree in human services, healthcare, business, or a related field preferred.
• Two or more years of caregiving, home health, employee performance, coaching, training, supervision, or related experience required.
• Experience with caregiver mentoring, performance conversations, documentation review, or compliance processes preferred.
• Comfort using digital systems for evaluations, tracking, reporting, documentation, and communication.
• Highly self-motivated, self-disciplined, organized, and able to manage a high-volume schedule with minimal supervision.
• Exceptional attention to detail and consistent follow-through on documentation, deadlines, and follow-up actions.
• Clear, compassionate communicator with strong active-listening, coaching, and conflict-resolution skills.
• Professional, fair, confidential, and comfortable holding standards while recognizing strong performance.
• Collaborative, solution-oriented, resilient, and committed to positive outcomes for patients, caregivers, and the organization.

• Tech-savvy; comfortable using digital evaluation forms, tracking tools, and communication platforms.
• Minimum of 1 years of caregiver service experience
• Experience handling multi-line phone systems and/or switchboards
• Strong multitasking abilities with clear and professional communication skills
• Ability to type at least 40 words per minute (WPM)

Nice To Haves

Responsibilities

• Conduct timely quarterly, six-month, annual, and as-needed caregiver evaluations in accordance with Impact Home Health standards and applicable Kansas HCBS, company, and payer requirements.
• Review relevant documentation, attendance and reliability patterns, patient and family feedback, supervisory input, and performance history before completing evaluations.
• Evaluate caregiver performance against clear expectations for professionalism, patient-centered service, punctuality, communication, documentation, safety, and compliance.
• Deliver evaluation conversations in a constructive, respectful, strength-based manner that recognizes achievements and establishes specific next steps for growth.
• Build positive, trust-based relationships with caregivers while maintaining appropriate professional boundaries and confidentiality.
• Recognize milestones, exceptional performance, positive patient feedback, anniversaries, and examples of values-aligned care.
• Identify engagement trends, morale concerns, coaching opportunities, and barriers to retention; communicate actionable insights to leadership.
• Support consistent employee engagement initiatives that reinforce belonging, growth, accountability, and Impact Home Health’s Culture of Care.
• Maintain complete, accurate, and timely evaluation records; upload documentation and supporting materials to the appropriate system within required timeframes.
• Track evaluation due dates, completion status, action items, corrective coaching, and follow-up commitments with exceptional attention to detail.
• Escalate performance, documentation, conduct, or compliance concerns promptly to Human Resources, Staffing, and leadership as appropriate.
• Partner with Human Resources and Advocate Caregivers to support coaching plans, recognition, disciplinary actions, and documentation that is fair, consistent, and well-supported.
• Collaborate closely with Staffing to understand patient assignments, schedule patterns, call-off trends, caregiver fit, and service concerns that may affect performance.
• Coordinate with Advocate Caregivers, Clinical, HR, and Operations to ensure caregivers receive aligned expectations, timely feedback, and appropriate support.
• Participate in occasional field visits or caregiver meetings when needed to complete evaluations, provide support, verify expectations, or resolve concerns.
• Contribute to continuous improvement by identifying recurring issues and recommending practical solutions that strengthen quality, compliance, and caregiver experience.
