About The Position

The Social Worker intervenes with patients who have complex psychosocial needs, require assistance with eligibility determination for social programs and funding sources and qualify for community assistance from a variety of special funds and agencies. This position is in person, onsite with requirement to assist other MCHS Emergency Departments virtually through a telehealth platform. In addition, the ED Social Worker offers crisis intervention to patients and families with psychosocial needs and coordinates and facilitates the development of a discharge plan of care for high-risk patient populations and will perform behavioral health/lethality assessments, make recommendations, and place patients in inpatient psychiatric hospitals as indicated. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the patient served. Must demonstrate knowledge of the principles of growth and development as it relates to the different life cycles.

Requirements

  • Master's degree in social work from an accredited program.
  • LISW license in the State of Ohio required.
  • Two years minimum of recent discharge planning experience in an acute care setting.
  • Knowledge of basic medical terminology.
  • Basic computer skills.
