This position provides Mental Health Screener functions and performs psychosocial assessments including collections of historical data, mental status assessments, and suicide risk assessments. It involves consulting and collaborating with medical center staff to coordinate interdepartmental patient services, developing comprehensive and realistic treatment plans based on patient's needs, and providing short term follow-up to ensure linkage and stabilization. The role also includes advocacy and support to patients to secure treatment and services, engaging families, treatment providers, and other parties active in the well-being of the patient whenever possible, and providing technical support, assistance, and education to patients, families, and other patient service providers.

Responsibilities

  • Provides Mental Health Screener functions
  • Performs psychosocial assessments including collections of historical data, mental status assessments, and suicide risk assessments
  • Consults and collaborates with medical center staff to coordinate interdepartmental patient services
  • Develops comprehensive and realistic treatment plans based on patient's needs
  • Provides short term follow-up to ensure linkage and stabilization
  • Provides advocacy and support to patients to secure treatment and services
  • Engages families, treatment providers, and other parties active in the well-being of the patient whenever possible
  • Provides technical support, assistance, and education to patients, families, and other patient service providers
  • Assumes the duties of charge person when required
  • Assumes the duties of mobile outreach on-call when required
  • Documents all telephone triage information; clinical assistment data; changes in mental status; and all communications related to patient referrals, transfers, and admissions in the patient record
