About The Position

Baystate Health is seeking an experienced Crisis Clinician to join its Emergency Department team. The role is based at Baystate Medical Center in Springfield, Massachusetts, and requires travel to support the emergency departments at Baystate Noble Hospital in Westfield and Baystate Wing Hospital in Palmer. The clinician conducts psychosocial and risk assessments, gathers information from patients and collateral sources, assists with legal documentation, supports quality assurance and outcome tracking, and develops discharge plans. The position is full-time, 40 hours per week, with eight-hour day shifts scheduled flexibly between 5 a.m. and 3 p.m. One weekend per month and a rotating A/B holiday schedule are required.

Requirements

• Master's degree.
• LICSW or eligibility for licensure.
• Experience in a behavioral health setting.
• Must be able to travel to support Baystate Noble Hospital ED in Westfield, MA, and Baystate Wing Hospital ED in Palmer, MA.

Responsibilities

• Obtain and maintain releases of information and gather collateral information from family, friends, providers, and other community resources.
• Complete psychosocial evaluations, including current and past history, mental status assessments, and risk assessments, using patient interviews, collateral contacts, and record review.
• Monitor and assist with completing legal documentation, including CVs, 3-Day Notices, Section 12s, and Section 7/8 forms.
• Enter data and review charts for quality assurance, program statistics, patient progress tracking, and clinical outcome tracking as assigned.
• Complete discharge plans addressing follow-up recommendations, step-down or placement options, transportation issues, and other treatment needs.
• Perform other duties and responsibilities as required.
