About The Position

Hamilton Pointe Health & Rehabilitation is seeking a detail-oriented, organized RN or LPN to join their team as an Electronic Medical Records Coordinator. This role is ideal for someone who loves to teach, train, and conduct audits, and wants to utilize their nursing knowledge in a different capacity. The EMR Coordinator will support accurate clinical documentation, educate nursing staff, perform audits, and help maintain high standards of resident care.

Requirements

Current RN or LPN license
Strong attention to detail and organizational skills
Excellent communication and teaching abilities
Comfortable working with electronic medical records and technology
Ability to identify concerns and provide constructive education
Strong knowledge of nursing documentation
Must possess a nursing license (RN or LPN)
Have a thorough understanding of the principles of best medical record practices
The ability to follow written and verbal instructions, function independently and make independent decisions when circumstances warrant such action
Personal integrity and the ability to deal tactfully with residents, family members, visitors, government agencies and personnel
The ability to adhere to professional code of ethics
The ability to relate information concerning a resident’s condition to the appropriate staff members

Responsibilities

Providing education and training to nursing staff on electronic medical records and documentation
Completing routine clinical documentation audits
Identifying documentation concerns and opportunities for improvement
Providing follow-up education and support to nursing staff
Monitoring records for accuracy, completion, and compliance
Assisting new nurses with EMR training and documentation expectations
Working collaboratively with the DON and other members of the leadership team
Helping ensure documentation supports quality resident care and regulatory requirements
Maintaining organized audit and education records
Coordinating, maintenance and administration of the clinical records program according to the accepted record-keeping practices of the facility in compliance with state and federal regulations and training and oversight of the Electronic Medical Records.
Monitoring records for completeness, accuracy, timeliness and internal consistency while the resident is in the facility
Assuring that all medical record information is protected, kept confidential and from loss or destruction
Training of all new employees on the electronic medical record data entry and procedures
Auditing records of new admissions, in-house residents and discharge residents according to established time frames, using proper forms and establishing method of follow-up
Developing and implementing a system for advising all disciplines of documentation to be completed or corrected
Working with the team to keep all medical records up to date
Assembling discharge records according to facility format such as obtaining the transfer information necessary for the continuity of care purpose form referring health care providers as needed for specific residents
