About The Position

Provide programmatic technical assistance to case management and serve as educational point of contact for provider and partnering agencies.

Requirements

  • Experience in Microsoft Office and Excel.
  • Ability to present trainings to Case Managers and Providers.
  • Ability to assess and problem solve quickly and make decisions.
  • Ability to assess appropriate intervention strategies.
  • Ability to work professionally with social service agencies.
  • Ability to work independently and with direct, onsite supervision.
  • Experience scheduling, conducting and leading virtual meetings.
  • Ability to maintain confidentiality of sensitive data.
  • Time management and organizational skills a must.
  • Ability to speak, read and write English at the college level.
  • Bachelor's Degree required.

Nice To Haves

  • Minimum of one (1) year of case management, child protective investigations, or school system, preferred.
  • Child Welfare Professional Credentialing preferred.
  • Bachelor’s degree from an accredited college or university in human services related field preferred.
  • Possess a current Background Clearance Screening Letter as required by DCF.
  • If local travel is required, a Valid Florida driver’s license and documentation of current automobile insurance, with $100,000/$300,000 bodily injury liability limit, is required.

Responsibilities

  • Provide consultation and support to case management staff in working with the educational system.
  • Assist in maintaining accurate records in Florida Safe Families Network (FSFN), by ensuring documentation related to education is entered and uploaded within 48 hours.
  • Provide the school districts with a current list of dependency children and a list of CPC employees monthly.
  • Obtain all school records as needed and provide them to case management (attendance, report cards, IEP’s, evaluations).
  • Serve as the single point of access for Volusia, Flagler and Putnam County schools for information sharing and the Every Student Succeeds Act (ESSA).
  • Coordinate, schedule, and lead all ESSA staffing’s. Completion of staffing’s and documentation should be finalized within 48 hours of the ESSA request.
  • Actively participate in Educational Workgroups and other educational initiatives, as directed by supervisor.
  • Assist in assigning educational surrogates, when applicable.
  • Collect documents/records and conduct thorough reviews for a qualifying diagnosis as it relates to 504 plans and IEP’s.
  • Collaborate with schools for necessary supports in the school setting; identifying further evaluations and therapies (if needed) and communicate with case management/clinical to accomplish needed evals and therapies. If supports require outside resources, actions steps should be taken for successful outcome measures.
  • Work closely with agency departments on educational outcomes and findings. Communicating often and efficiently.
  • Communicate daycare absences to case management and document in FSFN, same business day, in accordance with the Riyla-Wilson Act.
  • Ability to be cross trained with Children’s Disability Coordinator position.
  • Knowledgeable of child welfare and case management systems preferred.
  • Perform special projects as assigned.
  • Other duties as assigned.
