About The Position

The navigator helps patients access evidence-based treatment for substance use disorders (SUD) with a focus on access to medication for addiction treatment (MAT) and harm reduction. Navigators conduct outreach and identification of patients, conduct initial brief assessments, introduce patients to MAT programs and services, facilitate appointments at outpatient MAT clinics, serve as a resource and coach for their clients, and advocate for culture change in the hospital. They also assist with access to other services, such as mental health services, primary care, social services, and residential treatment facilities. Through skilled assessment, education, collaboration and coordination of healthcare and community resources, the MAT Navigator assists patients to gain self-efficacy/management skills, achieve optimum functional health status, and quality of life.

Requirements

  • High School diploma
  • Knowledge of and experience with Electronic Health Records
  • Good working knowledge of CPT and ICD-10 Coding
  • Current BLS certifications through any International Liaison Committee on Resuscitation (ILCOR) provided courses such as but not limited to: Ochsner Health System, American Heart Association, and American Red Cross. The ILCOR course must include a manikin check-off.
  • Must be able to maintain composure under difficult and stressful conditions
  • Must be able to maintain confidentiality and be able to work independently with little supervision
  • Displays tact and diplomacy in inter-personal skills
  • Demonstrates exceptional communication and negotiation skills
  • Demonstrates organizational, prioritization and time management skills
  • Self-motivated requires minimal supervision and ability to work independently
  • Strong interview, assessment, and critical thinking skills
  • Computer skills in Word, XCEL, MS Office, Outlook, Electronic Health Record

Nice To Haves

  • Clinical healthcare experience preferred with at least 2 years in hospital and/or ambulatory settings
  • Associate Degree in health care related field preferred
