About The Position

Under the direction of the CalAIM Director and ECM Program Manager, the ECM Registered Nurse (RN) provides clinical leadership and supervision to Licensed Vocational Nurses (LVNs) and Lead Care Managers (LCMs). The RN delivers clinical oversight and guidance to support high-quality, person-centered Enhanced Care Management (ECM) services. This role works closely with the ECM care team to implement and evaluate care coordination workflows, ensuring effective service delivery and compliance with program standards. The ECM RN may also lead multidisciplinary staff and support the development of policies, procedures, and program improvements. The position oversees health and psychosocial education, care coordination, patient advocacy, and documentation, ensuring services are delivered efficiently and aligned with organizational goals. Requirements

  • Valid CA Registered Nurse license.
  • 3-5 years' experience working as a Registered Nurse
  • Working knowledge of health information systems and data needed to monitor/assess utilization, performance management, and health outcomes.
  • Experience preparing and delivering presentations.
  • Extensive knowledge of development of evaluation tools, and evaluation of health programs, data analysis, and report writing.
  • Strong knowledge of community health evaluation tools, approaches, frameworks, and methods;

Nice To Haves

  • Bachelor of Science degree in nursing, medical, social work, psychology, public health, or related field preferred.
  • Experience managing a team preferred
  • Bilingual English/Spanish preferred (read, write, speak)

Responsibilities

  • Provide overall clinical oversight of the CalAIM Enhanced Care Management (ECM) program, including clinical day-to-day operations, service delivery, quality metrics, reporting, and performance feedback.
  • Oversee clinical workflows and ensure high-quality, compliant service provision across ECM teams.
  • Lead and participate in Multidisciplinary Team (MDT) meetings to support coordinated, patient-centered care planning.
  • Collaborate with internal and external stakeholders, including LVNs, RNs, ECM Program Manager, Lead Care Managers, Outreach Specialists, SJCH medical providers, and partner agencies.
  • Partner with homeless service providers and community organizations to support comprehensive care plans and address social determinants of health.
  • Provide clinical mentorship, supervision, and ongoing support to staff.
  • Support program infrastructure development, including coordination with SJCH leadership, providers, and community partners to enhance program effectiveness.
  • Maintain current knowledge of community resources and referral networks to support patient needs.
  • Ensure accurate and timely documentation in eClinicalWorks (eCW).
  • Review and audit clinical documentation to ensure compliance with Managed Care Plan and DHCS requirements.
  • Monitor and support program evaluation activities, including tracking outcomes and identifying improvement opportunities.
  • Develop and deliver training, presentations, and technical assistance tools for staff and providers.
  • Assist in creating educational and promotional materials to support program awareness and engagement.
  • Collaborate across departments to support strategic initiatives and integrate ECM services into clinical workflows.
  • Attend meetings, trainings, and other work-related events as required.
  • Perform other duties as assigned based on business needs.
