About The Position

The Lead Care Manager is a direct service provider for individuals referred through the Managed Care plan for Enhanced Care Management. The Lead Care Manager serves as the client’s primary point of contact and works with all their providers – such as doctors, specialists, pharmacists, social service providers, and others – to make sure everyone is in agreement about the client’s needs and care. This individual is responsible for assisting clients with navigating various systems to provide a Whole Person Care approach to reducing barriers and disparities with vulnerable populations. This individual ensures that services are: community-based, interdisciplinary, high-touch and person-centered. The Lead Care Manager is responsible for assisting and advocating for clients and their families who would benefit from access to resources in the community. These services include but are not limited to linkage to mental health services, medical care, educational services, pre-vocational services, vocational services, rehabilitative services, substance use treatment, housing and other community supports. The Lead Care Manager also assists clients to reach their goals by providing rehabilitative, case management, collateral, and consultation services. The Lead Care Manager is an energetic self-starter who can function comfortably in a team environment or independently and relates well to co-workers and external representatives. This position requires flexibility and the ability to manage time and multiple tasks. The individual in this position must be able to assume a wide range of responsibilities, provide timely responses to requests and deadlines and works well under pressure. The Lead Care Manager must demonstrate positive business ethics, communication skills, initiative, and good judgment. The Lead Care Manager must be able to take on a leadership role and have creative problem-solving abilities. The Lead Care Manager must be comfortable with public speaking and encouraging others to participate in meetings. The Lead Care Manager needs to be flexible, have a positive attitude, and be able to work collaboratively with others.

Requirements

  • Master’s degree (M.A./M.S.) in psychology, social work, public health or related field with at least 1 year of experience working with children and/or families.
  • Or Bachelor’s degree (B.A.) in social services or related field, with at least 2 years of experience working with children and/or families.
  • Other than BA degree with 2 years direct service experience working in case management or related experience in the field.
  • Proficient knowledge of: Electronic Health Record, Microsoft Word, Outlook and Excel.
  • Bilingual Spanish required (verbal and written skillset).
  • Valid California driver’s license.
  • Current automobile insurance.
  • Have and maintain a clean driving record acceptable to the organization’s insurance company.
  • Have immediate access to his or her vehicle during work hours.
