About The Position

This role is open to current employees on the Compass Family Resource Center (CFRC) team only. Compass Family Services is a 100+ year old nonprofit working on the front lines of San Francisco’s homeless crisis, providing families experiencing or at risk of homelessness with comprehensive support to address immediate needs and ensure long-term success. The Compass Family Resource Center (FRC) offers a comprehensive set of trauma-informed services that support family well-being. Homeless and at-risk families can access immediate crisis support and get connected with parenting education, support groups and classes, case management, housing search, and workforce services and housing support. This program offers new Medi-Cal reimbursable case management services called Enhanced Care Management (ECM), which provide intensive, whole-person care management to families with complex needs, connecting members to medical, behavioral health, and social services to support long-term stability and well-being.

Requirements

  • Current employee of Compass Family Services on the Compass Family Resource Center team
  • Minimum two (2) years of experience in case management, care coordination, or community-based services with families and/or individuals experiencing homelessness, child welfare involvement, or perinatal needs.
  • Demonstrated understanding of issues facing homeless and low-income families in San Francisco.
  • Experience working in a community-based, field-based, or non-traditional service setting.
  • Strong interpersonal skills, including the ability to develop trusting relationships with individuals and families across diverse cultural, linguistic, and socioeconomic backgrounds.
  • Proficiency with technology, including Microsoft Office, Google Suite, and database or case management systems.
  • Outstanding work habits: reliability, punctuality, organization, professionalism, and ability to manage multiple priorities independently.
  • Demonstrated commitment to diversity, equity, inclusion, and belonging.

Nice To Haves

  • Experience with Medi-Cal systems, CalAIM, or Enhanced Care Management programs.
  • Familiarity with trauma-informed care, motivational interviewing, and harm reduction approaches.
  • Experience with Electronic Health Records (EHR).
  • Community Health Worker (CHW) certification or Peer Support Specialist experience.
  • Knowledge of San Francisco community resources, service systems, and healthcare landscape.
  • Experience with Salesforce, Epic, or similar platforms is a plus.
  • Valid California Driver’s License preferred.

Responsibilities

  • Carry a dedicated ECM caseload, serving as the primary point of contact for families enrolled in Medi-Cal
  • Conduct comprehensive, whole-person assessments covering medical, behavioral health, housing, social, and economic needs in partnership with the Clinical Consultant.
  • Develop individualized, culturally and linguistically appropriate care plans in collaboration with families, caregivers, healthcare providers, and the Clinical Consultant.
  • Monitor care plan progress, update goals as member circumstances change, and support appropriate transitions out of ECM.
  • Coordinate services across medical, behavioral health, housing, and community systems to ensure integrated, seamless care.
  • Screen all new FRC referrals for ECM eligibility; enroll Medi-Cal eligible members and maintain accurate enrollment records.
  • Accompany families to appointments and provide health education, coaching, and motivational support as needed.
  • Serve as a member advocate with healthcare providers, systems, and community partners, with family consent.
  • Maintain accurate, complete, and timely case records in compliance with Full Circle Health Network contract requirements, DHCS standards, and Compass policies.
  • Develop and maintain relationships with healthcare providers, social service agencies, and community organizations to support referrals and care coordination.
