Bayview Hunters Point Foundation (BVHPF) has been a cornerstone of social justice for over 50 years, dedicated to serving predominantly low-income people of color in the Bayview and across San Francisco. Our mission is to foster an empowered, clean, safe, and healthy community through multidisciplinary, community-informed support. We address the root causes of unemployment and homelessness, advocating for essential human rights such as food, health, economic, and housing security. With nearly 200 employees, BVHPF serves over 5,000 disadvantaged clients annually through a range of programs including mental health counseling, substance abuse services, various housing programs, case management, and the Bayview SAFE Navigation Center. These programs are supported by nearly 20 contracts with the City & County of San Francisco and are subject to oversight from CCSF, state, and federal authorities. The ECM and JI Services Lead Care Manager (LCM) is pivotal in delivering comprehensive, member-centered case management for individuals enrolled in the Enhanced Care Management (ECM) program at BVHPF, with a specific focus on those eligible for Justice-Involved (JI) Services. The LCM manages a full caseload, facilitates access to health and social services, and collaborates with internal teams, healthcare providers, and community partners to enhance member health and well-being. This role reports directly to the ECM and JI Services Program Manager. The ideal candidate possesses direct case management experience, a solid understanding of CalAIM and Medi-Cal requirements, and a deep commitment to serving high-need and underserved communities.
