About The Position

The MCCN Clinical Coordinator will assist Mosaic Life Care providers in requesting an e-Consult as part of the Mayo Clinic Care Network (MCCN) membership. In addition, the nurse will submit referrals and coordinate all clinical aspects of the MCCN relationship. This person will also be responsible for educating all providers about the clinical tools available through MCCN. This role is vital to the overall success of the membership and to providing the absolute best care possible to our patients.

Requirements

3 Years - Clinical experience - Required
Registered Nurse (RN) - State Licensure/Or Compact State Licensure - Required Upon Hire
Hand/eye coordination; speaks clearly; good hearing; computer skills/telephone skills and voice recorder skills. Ability to read, write, and speak clearly.
Must be able to teach, provide accurate and concise information, answer questions, demonstrate excellent leadership skills, maintain positive relationships with multiple customers, motivate and persuade others.
Physical requirements include constant standing, walking, and mobility of fingering and fine motor skills; Frequently reach for items; Occasionally bend, squat, sit, and climb stairs.
Sufficient strength and mobility to sit for long periods of time (depending on assignment), written reference and resource material.
Appropriate functional vision and hearing acuity.
May have to drive; frequency would depend upon specialty assignment ie: home visits, outreach/educational events, travel to various campus locations, etc.
Critical thinking skills, problem-solving skills, negotiating abilities.
Capable of determining appropriate response to emergency situations.
Ability to focus and concentrate in order to correctly perform tasks despite numerous distractions and interruptions.
Ability to maintain professional demeanor to communicate and interact with patients, customers and team members.
Appropriate functional vision, hearing acuity, and smell.
Driving to and from various locations for the purpose of coordinating patient care/collaboration and outreach.
Respiratory exposures for airborne pathogens.
Exposure to infectious pathogens.
Exposure to physical hazards.
Frequent contact with water/soap/detergents.
Potential of assault or unexpected hazards.
Potential to make home visits.

Responsibilities

Have expert knowledge of the MCCN and educate providers and caregivers on the relationship between Mosaic and Mayo Clinic.
Submit eConsults to Mayo Clinic via Carelink
Submit referrals to Mayo Clinic via Carelink
Collaborate with Marketing to educate the community, caregivers and providers.
Complete administrative duties as needed to keep Carelink up to date with current providers.
Collaborate with Marcy George, the MCCN Business Coordinator, to plan the Annual Mayo Business Meeting and advise on any other MCCN clinical projects.
Meet monthly with Chief Medical Officer to discuss MCCN updates
Participate in MCCN Clinical Liaison meetings quarterly.
Participate in monthly MCCN Business Meetings with our MCCN Operations Manager.
Collaborate with the Mayo team via webinars, email communications, and direct communication to develop the most effective ways to use the MCCN tools.
Educate Mosaic Leadership and all providers verbally (via Board of Governer's meetings, Director's meetings, Quarterly Providers Meetings, etc) and written communication (via physician newsletter, email, etc.).
Assist in the implementation and use of any MCCN resource.
