About The Position

Change Grow Live is a charity dedicated to making a difference in the lives of their Service Users. They are looking for a specialist Dual Diagnosis (Co-existing Mental Health and Substance Use) Recovery Worker based in Preston and Central Lancashire, to join their Lancashire-wide team. This role involves working closely with community and inpatient mental health services to create joint recovery plans for individuals with complex co-existing mental health and substance use needs. Many clients also face challenges such as neurodevelopmental conditions, learning disabilities, unstable housing, poor health, safeguarding concerns, and limited support networks. The role is mobile and hands-on, requiring assertive outreach and in-reach support in various settings including GP surgeries, mental health services, substance misuse hubs, and supported accommodation. This position is suited for a resilient, empathetic, and motivated individual.

Requirements

  • Experience of working in substance misuse and/or mental health settings.
  • Experience of working with people with co-morbid mental health and substance misuse problems.
  • An understanding of and commitment to safeguarding best practice.
  • Experience of managing a caseload of clients with complex needs.
  • Knowledge of and skills in appropriate therapeutic interventions for working with people with dual diagnosis.
  • Knowledge of current mental health, substance misuse, and dual diagnosis policy, guidance, and legislation.
  • Ability to liaise and work in partnership with a wide range of professionals and agencies.
  • Excellent team working and interpersonal skills, maintaining a highly cooperative approach to supporting colleagues in delivering service objectives.
  • Must have the right to work in the UK.

Responsibilities

  • Provide bespoke interventions for identified service users, aiming to engage them in appropriate services, reduce re-presentations, and achieve positive client outcomes.
  • Support the development of joint working protocols with mental health services and substance misuse services (including CMHTs and Improving Access to Psychological Therapies services).
  • Identify training and support needs of colleagues working with this client group and deliver training or provide support as appropriate.
  • Identify gaps in dual diagnosis provision and provide examples of good practice to inform future pathway and service design.
  • Improve communication and referral pathways between substance misuse and mental health teams and other services involved in dual diagnosis care.
  • Provide case management for identified dual diagnosis service users as appropriate.
