About The Position

On Lok PACE is a non-profit organization dedicated to the care of elders through the Program of All-Inclusive Care for the Elderly (PACE). The PACE model focuses on serving seniors with chronic care needs in their community. Participants receive in-home care services and transportation to a PACE center for medical care, social activities, and other senior care services. The organization aims to improve lives and advance healthcare by providing a system that works together. The Driver will provide transportation services to participants and assist with deliveries and pickups as needed.

Requirements

  • Licensed California Driver with a good driving record.
  • Ability to operate all vehicles and possess some mechanical ability to understand vehicle maintenance programs.
  • Understand and carry out oral and written directions.
  • Familiarity with traffic routes and driving regulations in San Francisco.
  • Ability to lift 50 pounds.

Nice To Haves

  • One-year experience working with the frail or elderly population.
  • Flexibility and creativity; ability to get along well with frail elderly people and exercise mature judgment.
  • Bilingual English/Spanish or Cantonese/English.

Responsibilities

  • Safe operation of vehicles, including proper use of wheelchair tie-downs and lifts, while transporting participants from their homes to Day Health Centers and community appointments.
  • Ensure participant and vehicle safety through van maintenance, pre-trip inspections, use of seatbelts, and adherence to body mechanics, transfer training, and wheelchair safety protocols.
  • Provide timely and safe transport of participants, meals, and supplies as scheduled or requested between home and center, and to community appointments.
  • Support participant's care plan by assisting with transport, making observations, and reporting to the Interdisciplinary Team (IDT).
  • Perform other related duties as assigned.
