About The Position

The Patient Navigator helps ensure that the biopsychosocial continuum of care needs of Callen-Lorde patients are identified, and appropriate services accessed, coordinated, and monitored. The Patient Navigator is responsible for providing field based case management and adherence support services to HIV-positive individuals accessing medical services. The Patient Navigator will work in a team setting headed by a Care Coordinator, in conjunction with medical and mental health providers, to facilitate HIV positive patients’ retention and adherence to primary medical care.

Requirements

• High School education
• Ability to work effectively with diverse populations, including gay, bisexual, and transgender communities
• Ability to work within team-based structure
• Strong communication and interpersonal skills
• Commitment to the mission of Callen-Lorde

Nice To Haves

• College degree (BA/BS) in health/social services preferred
• Experience in social services, case management and/or entitlements work preferred
• Computer literacy preferred
• Bilingual (Spanish/English) preferred
