About The Position

Malama leverages the best of technology and people to deliver exceptional care - we build novel biometric tracking and patient engagement tools that allow skilled Care Managers to provide high-touch care navigation and coordination for pregnant and postpartum mothers that improves health outcomes for mom and baby. We’ve raised $9.2M in funding to date and are on target to hit profitability early this year. Our mission is to improve pregnancy-related outcomes beginning with conditions that disproportionately affect historically underserved groups. We're seeking Community Health Worker or Community Doula to use our tools and serve our members as their Maternity Care Navigators. This role is pivotal in bridging the gap between our providers and patients, ensuring seamless execution of our policies and care protocols. Requirements

  • 1-2 years of experience as a Community Health Worker, Community Doula, Case/Care Manager, Social Worker
  • Must be a certified birth doula prior to employment, or commit to completing Malama-sponsored doula training (training, attend 3 births, complete CPR training, etc) after onboarding and enroll in DHCS Medicaid
  • Reliable and responsive to text and calls!
  • Adaptable and Excited to learn new technology - You are comfortable using Google Workspace (Sheets, Docs, Drive), Notion, Slack, Zoom, and want to learn more

  • You are fluent in English (written and verbal) and bilingual in Spanish, Mandarin, Cantonese, Fuzhounese, Bengali, Arabic, Haitian Creole, Russian, or Albanian
  • Located in New York - preference for the following counties: Kings, Bronx, Queens, Nassau, Westchester

Responsibilities

  • Conduct outreach, check-ins, and comprehensive assessments to identify and meet patient needs via phone calls, SMS, video calls, or in-person encounters (based on patient preference); Assist patients with appointment setting, office visits, social services application, among other requests
  • Advocate for patient needs by coordinating with health plans, service providers, multi-disciplinary care teams to drive services to completion
  • Utilize evidence-based practices like motivational interviewing, trauma-informed care, and harm reduction to engage with and communicate information to patients towards positive habit change
  • Utilize required care management portals for all care coordination and other services provided in a timely manner; meet patient caseload targets and prioritize services according to intensity, need, and required follow-up.
