In this role, you will coordinate child and family team meetings, manage new client intakes, help participants access essential community resources, and provide direct support to individuals navigating trauma and crisis situations. You will collaborate with multidisciplinary teams, maintain ongoing communication with participants and families, monitor progress, and ensure treatment plans are implemented effectively.

Requirements

  • Bachelor’s degree in a Human Services field from an accredited college or university
  • Minimum of two (2) years of relevant work experience
  • Must be at least 21 years of age to operate a company‑owned vehicle and at least 23 years of age to operate a 15‑passenger company van
  • Must have a valid driver’s license, acceptable driving record, and current auto insurance
  • Ability to maintain confidentiality and communicate effectively with clients, families, team members, and community partners
  • Proficiency in computer and Google applications
  • Strong judgment, time management, and interpersonal skills
  • Ability to manage multiple tasks and adapt to changing situations
  • Ability to build positive relationships with associates, clients, and the public
  • Must attain First Aid and CPR certifications within three (3) months of employment and maintain upon renewal
  • Successful completion of Behavioral Management training within 30 days or first available class

Responsibilities

  • Facilitate child and family team meetings to support individualized treatment planning and service coordination
  • Provide compassionate case management services, including connecting individuals and families with essential resources and support systems
  • Collaborate with multidisciplinary team members to develop and implement Comprehensive Treatment Plans tailored to participant needs
  • Coordinate referrals, crisis management efforts, and advocacy within community systems
  • Conduct client intakes and assessments to inform individualized plans and ensure smooth onboarding of new participants
  • Maintain regular communication with program participants, serving as a reliable liaison between clients and team members
  • Monitor participant progress and update treatment teams regarding goal achievement and emerging needs
  • Assist individuals in identifying and accessing services that promote safe, independent, and healthy living
  • Provide direct daily living support and case management to children and families
  • Arrange transportation to community appointments when necessary
  • Participate in organizational processes, including quality assurance and safety activities, as directed by leadership
  • Evaluate services and recommend improvements to enhance participant outcomes
