About The Position

Under direct supervision, responsible for assisting with the screening and evaluation of clinical documents submitted for additon into the patient medical record. Facilitates and completes the preparation, scanning and indexing of all patient medical record documents.

Requirements

  • Knowledge of medical terminology, clinical documents and electronic records applications.
  • Self-motivated and directed; takes ownership for deliverables and deadlines.
  • Strong attention to detail and accuracy.
  • Ability to work with a team to meet productivity standards.
  • Ability to recognize clinical results and attach to the correct patient, encounter, or physician order.
  • EXPERIENCE: One (1) year related experience with at least one year medical records or document imaging experience is required.
  • EDUCATION: High school diploma or GED is required

Responsibilities

  • Prepares, scans, and indexes paper documents for accurately filing to the patient’s electronic health record.
  • Ensures all received clinical documents are accurately imaged and validated to patient’s identification.
  • Identifies all documents readied for scanning process prior to performing the imaging process.
  • Ensures the appropriate patient and related documents are attached and matched prior to scan.
  • Prepares paper medical records and single clinical documents for document imaging to include patient look up for medical record number, removing staples and clips, repairing torn documents, and tracking all received documents/records for scanning in an assigned format.
  • Indexes scanned documents by entering brief and concise description and ensures appropriate document type selection as designated by department rules.
  • Ensures all imaged clinical documents are bundled and boxed with clinic identification and imaged date(s) in preparation for destruction.
  • Validates that patient labels match the name on the clinical document to ensure accurate patient records in EpiCare and the Health Information Exchange (HIE).
  • Performs corrections as needed to ensure data integrity.
  • Ensures all clinical documents are accurately imaged and assigned to the correct clinical department PID for billing.
  • Responsible for attaching lab, diagnostic imaging, and procedural results to the corresponding physician order in EpiCare and alerting the physician of results.
  • Creates Health Maintenance orders in EpiCare essential for quality metrics and reporting.
  • Reviews documents and submits for correction when needed.
  • Protects patient information in accordance with HIPAA rules and regulations / Federal Guidelines.
  • Performs quality review of indexed documents.
  • Submits errors to supervisor for correction.
  • Performs all other duties as assigned.
