Columbus House, Inc. is seeking a full-time DMHAS Outreach & Engagement Case Manager to support and link consumers to housing, educational, employment, vocational, spiritual, recreational, medical, and treatment resources. The objective is to reduce the numbers of men and women in Middlesex County who struggle with mental illness and/or substance abuse which has or can lead them to homelessness. This role involves engaging in street outreach to consumers who have histories of homelessness, mental illness, substance abuse, unemployment, incomplete educations, lack of employment skills and are disengaged from services. The case manager must be comfortable providing a full range of case management services to individuals in the community, i.e. in soup kitchens, shelters and on the streets. The position requires collaboration with the CAN Entry and Exit process in the Middlesex region and may require working one evening a week at the local Warming Center during the winter season after 9 PM. Accurate and timely documentation is essential, including entering client data into the Case Worthy computer program.
